Visitation and Memorial Service With Cremation

• Visitation 1-2 hours before Memorial Service at funeral home or church
• Evans Funeral Home professional services and staff, use of facilities and automotive equipment, and printed materials

Visitation and Graveside Service With Cremation

• Visitation 1-2 hours at funeral home with procession and graveside service at cemetery
• Evans Funeral Home professional services and staff, use of facilities and automotive equipment, and printed materials

Immediate Burial or Direct Cremation

• These services involve no visitation or ceremonies
• Evans Funeral Home basic services; removal from place of death, transportation to cemetery or crematory
• Basic staff services involving obtaining legal authorizations and signatures and permits

The State of Ohio requires that this Authorization Form be completed and signed prior to the cremation. Please read it carefully and ask us any questions you may have. Cremation is an irreversible and final process. It is important that you understand the cremation process that is described in Section 9 of this Authorization Form prior to signing it.
